Are you talking about heart lung transplant? If so you need to contact the nearest major medical center that specializes in that kind of surgery and ask about indigent care programs.
Check with the Cleveland Clinic in Ohio

Please contact the Cleveland Clinic in Cleveland, Ohio and ask for an appointment with the Heart & Vascular Center. Also, talk to the social work office and tell them you want to apply for financial assistance. The forms you need to apply are on-line. You can also go on-line and see the other locations the clinic has (i.e. Canada, Florida and Minnesota). You can also ask them about housing arrangements such as Hospitality Homes (216) 518-0404 where the daily rate is $30/day.
Cleveland is #1 nationally for heart and I'm going there in 2 weeks. They have so many programs to help patients get the care they need both from in the US and out of the country.
I don't have insurance either and I'm using my tax refund for travel and lodging expenses. I've applied for financial assistance. The form isn't really long. The main # to the Cleveland campus is: (800) 223-2273. You can also request an appointment on-line if calling is not your forte. Email me if you like.
