Frequent Sinus Tachycardia

Hi all!
I just received a call from my EP nurse regarding the results from my 30 day monitor. She said my rhythms are normal and just have frequent ST’s. In the message she did not state if they were IST or just plain ST’s and I didn’t need a follow up visit until my regular scheduled appt. I have searched ST and understand they are not a big deal but wondering what the long term effects are. I have a PM from a AV node ablation, have tried many different settings. Because I have ST all day long I feel very tired at the end of the day and depending on how bad they are end up with chest pains at night.

For those of you with ST, what has worked best for you? I have a conservative EP but I don’t think he is apathetic to how I feel at the end of the day. I run on average 120-130 during the day but when I rest it comes down nicely to about 80. I do have a stressful job but stay away from stimulants.

Can ST cause any long term effects on the heart!  Don’t get me wrong, I am elated that my rhythms are normal but having a hard time just dealing with ST.

If you had an AVNA you should be having SVT, whether sinus or IST. That is— it should not be conducting to the ventricles.

Sorry to hear you are not feeling well, but at 120-130, I can understand why. Why did you have an AVNA (AF or AVNRT?) Here's a few thoughts; hope they help:

I'd want to look at the device since your rate is between 120-130— the URL/MSR of most pacemaker and your rate comes down when you rest, it sounds like a sensor issue. Is your lower rate limit 80? If you know your basic parameters (specifically, LRL, URL, MSR and sensor parameters). I may be able to give you some more feedback....

If you had an AVNA it's possible that the ablation did not take care of the reentry completely, which is not uncommon.

Hi Chili13,

Please forgive me I am not sure of some of the abbreviations you used but my settings are 60-180 with a sleep mode at 50. My rr is off. I had my upper setting at 135 but am a swimmer and had major issues even during regular activity having my PM push my rate down so I requested the increase with a 30 day monitor. Two months ago I was having SVT which is why I was surprised to hear that the reports all indicated normal ST. Again, I'm glad but not thrilled with feeling like I worked out all day long. Hope that info clarified my situation.

Sorry about the abbreviations. If your rate response is off then that rules that out. Since you've had an AV node ablation, you may want to consider asking the nurse/doc how your heart can be going that fast? Whether SVT or ST, makes no difference. Because, if the AVN was completely ablated; any signal originating from the from the atrium can not pass to the ventricle (at least not via the AVN), and you would be pacing. Hope you get to feeling better.
