The Drug Entresto

Since I have the combined issues of paroxysmal A-Fib, & Heart Failure (I'm an A-V Node ablated dude) - my PM is equipped with an ICD.  Cardio Doc. wants me to try Entresto.  My LVEF was 18% 5 years ago - my 1.5 to 2.0 hrs. everyday exercise regime has raised my EF to around 25%.  Anyone have any comments regarding Entresto?

This drug is approved in the UK by NICE, as cost effective if your ejection fraction, EF is < 35.

 It is the first treatment shown to offer a significant mortality benefit over an ACE-inhibitor; with data from the 8,442 patient PARADIGM-HF demonstrating that it cut cardiovascular deaths by 20% versus enalapril, as well as heart failure hospitalisations and all-cause mortality by 21% and 16%, respectively.

You need to have decent kidney function, and monitoring on a regular basis. 

You may wish to consider re-synchronisation therapy as you already have a PM and your EF is <35? ( I have just posted about this today elsewhere).

OK, test is over......I used the lowest dose of Entresto 24/26mg. Impression: Blood tests showed no renal issues developing. However, blood pressure would hang 88/54 range accompanied by lightheadedness. Few weeks ago, I drove wife to her clinic and a he-man dizzy event began just when I shut off the cars engine after parking. Wife went into the clinic and fetched a wheelchair to haul me in. No treatment was offerred. One hour later, I recovered. Coulda been interesting if this event happened 10 minutes earlier when I was traveling I-35 at 75 mph. Couple weeks later, another dizzy event occurred. I was just sitting & watching TV. It was a Friday evening, znd I called my pharmacist to get her instruction regarding weaning off Entresto and re-initiating Lisinopril. She said, let 36 hours pass between last Entresto and first Lisinopril. I've done so and no dizzy events since. My blood pressure hangs around 105/65. I've had numerous calls from Docs staff and Entresto's staff trying to urge me to continue use. Not gonna happen! I have a face to face appt. with my heart failure Doc in 2 weeks & I'll simply tell him Entresto is not for me!!

Pretty much follows this scenario: "Here! Take this drug and let's see what happens!"
