Where to get ID card?

Hey everyone,
So I've had a Pacemaker since I was little, and I turned 18 last year, but I never got a new ID card(since my old one says child/underage/dependent). I've never really needed to use my ID card before, but I feel like I should get a new one. And I don't have anyone to talk to about this since my mom died in June. I called my cardiologists office about it but all they told me was that I need to call the company, but I'm not sure what information I'll need to give them. I have a St Jude Pacemaker, and I'm not sure which St Jude number to call, since they have so many. Can anyone help me?

They are super nice and responsive! All you need is your name and maybe birth date, they can look up the rest. They can also answer questions about your device. I wanted to know how deep I could scuba dive and they were able to tell me right away. And when I lost my id card, they sent a new one after a fast phone call. (then I promptly found the old one so now I have two)

All you should need is identification information and perhaps an implant date. They have a database of who has what model.
