Usually it's low battery.  Recalls are rare but they happen and sometimes that means early replacement. If a lead went bad sometimes they'd give a new box when they fix the lead. Upgrading to a 3 lead or ICD.

Beside low battery I have had to have a replacement cause of a recall and also cause of my leads failed.  Had it since I was 17 and have had 5 replacements.

I had number four replaced early to get number five.  "upgraded" from a dual chamber to a three chamber (biventrical).   

Basically battery is the reason why they get replaced, with rare exceptions.
