Sharp pain

I had my dual chamber PM implanted 4 days ago after the procedure I started getting some sharp pains in my opposite shoulder. By that night it was so bad I was in tears. It has gotten better but now it is horrible shooting pain in my right shoulder with every heartbeat if my HR goes above about 100. Could this all be from swelling or the procedure, or do you think it's a lead placement issue? I'm calling the doctor on Monday if it persists.

If you are in enough pain to cause tears, call your doctor now.

That was a few days ago, we did call them at that point and the on call dr said it was normal. Now it's just when I get up and my HR goes above 100 or I lean forward and squat. Which makes me think it's due to a lead, but it could just be caused by swelling.

Pain in the shoulder *opposite* the side of the PM implant isn't trivial

I echo Tracey_E's advice.  Pain in the shoulder on the side *opposite* the PM implant suggests that your diaphragm is being irritated - especially if the pain is near or at the shoulder tip and it's worst when you lean forwards.

Don't ignore this and seek an urgent appointment with your EP doc.

In your shoes I would be pushing hard for a CT-scan to assess the position of the RV lead and state of the lung on the painful side.
