I didn't fill this out but is this serious or a scam? They ask you for personal info at the end so it makes me wonder if it's just a scam to get mailing/phone info and to telemarket/snail mail spam you.
I'm probably just being overly paranoid :-).

... but it seems odd that their server would be accessed using just an IP address, instead of a domain name. I looked up the IP address, and it is run by "secureserver.net". The domain name owner is not published, so I'm not sure who they are. I would not fill it out, personally.
