Need some advice!

Hi, haven't logged in a couple of months. So I would like to say Hi to all of you! Hope Jesse, Smitty ,Pacergirl, Peter Nash and many more are still here. I need some advice. I have been having some more problems. For some time I haven't been able to walk to one roon to the other or even carry my laptop without getting chest pressure and then pain and feeling like someone has there hand around my throat. Been back for several test. Haven't found anything new. I already have Idoopathic Restrictive Cardiomyapathy, Conjestive heart failure, a bad mitral value with +2 regurations,now a leaking tricuspid valve( think from the lead wire ) they took out my pericaridal sac because of the fluid build up. Then I got the pacemaker for Sick Sinus Syndrone and one time to ER and Medtronic's Rep said that I was in the wrong mode. That I had developed heart block in the ventricle and changed the mode to DDDR ( what ever that means). Was doing better then this started about the last 3 or 4 months. Both my cardilogists didn't really find anything they already know .Then for a last resort did a holter moniter for 24 hours. It said that all heart rate was pacemaker oriented. Needless to say I had the report faxed to me in 2 days and never heard from a doctor till I called to make an appt.a couple of weeks later. He said yes you were very symptomatic. I could have told him that. But didn't see anything different. Great! Getting really fed up at this point. So we talked about getting a second opinion maybe Cleveland or Mayo. Then we talked about cardiac rehab for the 3rd time. Even thought I couldn't walk even to the bathroom without getting so much pressure and if I tryed to stay up chest pain so bad I would take a nitro, I am thinking I give up I will try it before I go elsewhere. Have been in rehab for 1 month. It has made a huge difference. I only can do the sit down Nu-Step but slowly I am reaping the benfits. Ok, now that you have the pifcture. My big thing is I just had a pacemaker check and ask the rep can you tell me how much I am pacing? She said all the time! What ? I only was pacing 42 in the atrial and 53 in the ventricle last check up. Ok I ask later on the same question. Got the same answer. Shocked and didn't like a dumb dumb ask any more questions. So went to rehab two days later and ask one of the nurses there and she looked at my records and and said yea that everytime I had been there I was pacing about 90 to 95 % each time. I don't understand why I wasn't told this and even more why am I using the pacer so much? When I did see my cardio he didn't say anything that I was. So now I am wanting answers and I thought you were the best group of people to get it from. Why would I be pacing so much when I year June 20,2007 I got the PM for heartrate dropping and pausing 4 seconds at a time? I hope someone can help me and do I need to see am EP and if so what do they do? Also my blood pressure has been dropping so low I was almost passing out when getting up so they took me off of my ACE inhibitor to see if that helped. Then the next week the arrythmia med. Ethmozine I have been taking for 18 yrs, they are not going to make it anymore. So I had to just stop it cold turkey after only having enough for 2 more days when the pharmacy called my doctor. This all just happened about 2 weeks ago. Now I am have SVC which I had before but where under control. They said that the pacemaker would take care of anything that changes from going off the medicine. My heart beats so heard most of the time and it feels like at times it is sucking in when it beats and really feels strange. Just episodes though. Then feels normal whatever that is. Please help!

Thank you for your information. You can feel free to drag me through the mud. I can take it. At this point I am taking the attitude "Just suck it up" and go. That got me to cardiac rehab. Is is so wrong to just want some answers from the doctor? As my wonderful 83 old father always say" they just PRACTICE medicine"! Love it and he is so right! It really threw me for a loop when they said I was pacing that much! I know I am not the only one but I just want to know why. I guess I need to take the bull by the horns. LOL Good to hear from you! Hope you and wife are doing well.

Hi AMY,
Could you suggest to your Dr. that an EP study in an EP lab might be in order? Sometimes when they fool with the settings at a check up, they are trying for a quick fix, an educated [I hope!] guess, and a way to hustle you out of the office. When an EP study is done, the Dr. takes his time, and tries out different modes and settings that would probably benefit you better! You more or less have his/her undivided attention. Just a thought! Hi Amy,

I have to say you really have a plate full now. I wish I could be of help, but all I can do is guess and I don't think you need me to muddy the water for you anymore than it is now.

There is one thing I can comment on and is the DDDR setting on you pacemaker. What they did when they changed your PM from DDD mode to the DDDR mode is activate the Rate Response feature. In other words, when you start physical activity your PM will come on line and speed up your heart rate. How quickly and how much it will speed up the HR depends on the settings they have given you.

I'm familiar with the DDDR mode because this feature was activated on mine a couple of months ago. This is the second time I've had this done and MY results are just so, so at best.

My one suggestion for you is get that second opinion, if at all possible. You may learn nothing new, but I feel that you have so many unanswered questions that even getting the current information confirmed would provide you with peace of mind, if nothing else.

Amy,
With your list of cardiac problems, it isn't surprising that you are pacing a high percentage of the time. The good news is that pacing even 100% isn't all that bad in itself. I have A/V block and pace 100% in my ventricles and expect it.
The hard thing to accept is that a pacemaker, ablations, and meds are only ways of working around a heart problem. None of these fix the problem and there are limits to what can be done.
The only suggestion I can give you is to get a second opinion at one of the well known heart hospitals. They might catch something that has been missed or come up with a new approach. More important they might give you more confidence in your treatment. For me I always ask for a prognosis so I have some idea of what the future holds.

I am for one very happy to see you tonight. I was wondering what had become of you. Yes, in answer to your question I am still around. I am staying quite busy, with this and that.
Now about you, I don't know what to think, seems you certainly have a lot on your plate. Your attitude is fantastic! To be admired actually. I can tell you that I too am always surprised by the amount that i am pacing, It seems to be more and more. I am up to 95%! I would also like to know why? I don't know how to get better answers from the docs. I hope you find the answers you are seeking and are willing to share with me.
I really can't offer much help, but I am happy to see you have returned.

Bambi, thank so much for your suggestion. I am at the point that I don't want one more test or doctor to touch me. I know that you have been through a lot in the last few years that I have been a member so I am sure you and many others can relate to this. I know in my heart that I should see one. I am scared as to things just might get worse. I will and am considering it. My doctor from Ohio State thinks I probably should see one. He was in Italy doing a semiar when I got that info so I haven't got to talk with him for a visit. Hope you are doing better. How's the little one?

Pacing Percentages

Hi Amy,
I, like many, do not particularly like being dependent on my PM. My top half is 100% and my bottom half (last report) is 60%. In 2005, I did not start out that way.
It took about a year of "fine tuning" to get me where I am today. I'm not light-headed (very often) and I rarely get out of breath. My PM people would do one or two adujstments at a visit - the idea being that if they did several adjustments and once, they would not know which adjustment actually helped. Like, Bambi's comment, I think they need to be methodical and deliberate in fine tuning your PM to fit you and your needs.
Hang in there with your cardiac re-hab. It sounds like you are having sucess!
