7 weeks after my PM implantation

Hi my friends,

I was on another check-up yesterday, 7 weeks after my implantation. My doctor told me that I am pacing at 100% (V pacing). I have got third degree AV block - my atriums are still going very fast, but it never minds, because the ventricles are pacing by the PM.


My setting:

Base Rate - 60/min
Hysteresis Rate - Off
V. Refractory - 300 ms
V. AutoCapture - Off
E/R Sensitivity - 1,6 mV
V. Puls Amplitude - 2,1 V
V. Pulse Width - 0,31 ms
P/R Sensitivity - 3,0 mV
Vario - Off

SENSOR PARAMETERS:

Sensor - On
Max Sensor Rate - 140/min
Slope - 11
Reaction Time - Fast
Fast Response - On
Recovery Time - Medium

TEST RESULTS:

Ventricular Evoked Response
Measured Evoked Response - 4,44 mV
Safety Margin - 2,8:1@1,6mV
Measured Lead Polarization - < 1,0 mV
Safety Margin - >1,6:1@1,6mV
Auto Capture is not recommended.
The Lead Polarization safety margin is below (1,7 : 1).

R Sense Test
Measured P/R Signal - 12,75 mV


Anyone know what these settings mean?

my PM stimulate just my ventricles, I have got inappropriate sinus tachycardia with average heart beat about 130-140/min (during 24 hours holter monitoring) and then I have multifocal SV tachycardia (from 180 till 250/min) and one time I had ventricular arythmia as well, so I was on 5 RF ablations and at the end of the fifth ablation they had to destroy my AV node... So I have got third degree AV block now, so the ventricles are stimulated by the PM (V pacing is at 100%).

The VVIR mode that is shown indicates as you have said that you have ventricle only pacing. The R on the end means that the pacing rate is determined by the rate response sensor so your heart rate should respond to activity.

Max Sensor Rate - 140/min means that the upper pacing limit is 140bpm

Slope - 11 is how the the rate at which the HR responds to activity

Reaction Time - Fast tells the pacer how quickly to start responding to activity.

Fast Response - On I'm not sure on this one.

Recovery Time - Medium is quickly the HR slows back down after activity.

The bottom line with all this is to be aware that the pacemaker is setting the way your heart responds to exercise. If you find your heart doesn't seem to be reacting appropriately try to notice the conditions where there is a problem. Then let the person doing your checkups know and they can adjust it. It may take a few trys to get it right, but don't give up. It will never be like your natural heart, but it can sure come close.
