pain

I have a tear in my bicep muscle which is extremal painful, have tried pills creams physio,. no joy, would it be safe to use a tens machine, i know it states  not to use with a pacemaker, just wondered if any peeps on here have used one

For sure don't use on the torso but sometimes it's ok on extremities.

After I broke my writst, I used a TENS unit on my lower left arm without any ill effects, and would do so again. I only used low to moderate intensity settings, but the electrical stimulation didn't spread very far from the application site. For instance, I couldn't feel it in my upper arm--or anywhere else.

I probably wouldn't use one on my chest or upper back, neck or face--but anywhere else? I can't tell you what to do, but speaking for myself, I wouldn't hesitate.
