Twiddling is when a person plays around with the body of the pacemaker (called the "can") and moves it around in the pocket. Many times this can cause a break or fracture in the wire or flip the pacemaker over. Not a good thing.
Doctors will move the pacemaker body to see if the rate response is functioning.

Just gently wiggling the pacemaker up and down will stimulate the rate response and isn't hazardous. Twisting it around in the pocket could be rough on the leads.
